Psilocybin and triptans

No data

Migraine treatment, 5-HT1 agonist · also: sumatriptan, imigran, imitrex, rizatriptan, zolmitriptan

Psilocybin and triptans: low practical concern for most people, with the theoretical worry being blood vessels and the heart rather than serotonin syndrome.

No characteristic reported interaction. The concern is theoretical and mostly cardiovascular.

Why it happens

Triptans act at 5-HT1B/1D receptors on blood vessels and nerves and can narrow arteries, which is why they are contraindicated in coronary disease. Psilocin acts mainly at 5-HT2A but also binds 5-HT1A with moderate to high affinity. Any additive effect on blood vessels is theoretical.

What is known

None specific to this pair. The serotonin-syndrome warning attached to triptans generally is itself considered overstated by much of the headache literature.

2026
Sumatriptan binds with high affinity to 5-HT1B/1D receptors; it is contraindicated in ischaemic coronary disease and coronary vasospasm and carries a serotonin syndrome warning mainly for SSRIs, SNRIs, tricyclics and MAOIs. IMITREX (sumatriptan) tablets US prescribing information, DailyMed (label version Aug 2026)
2018
Among patients prescribed triptans with an SSRI or SNRI (30,928 person-years), only 2 had definite serotonin syndrome (0.6 per 10,000 person-years); the authors questioned the FDA warning. Orlova et al., 'Association of coprescription of triptan antimigraine drugs and SSRI or SNRI antidepressants with serotonin syndrome', JAMA Neurol
2011
Indoleamine hallucinogens, including psilocin, bind 5-HT1A receptors with moderate to high affinity. Halberstadt & Geyer, 'Multiple receptors contribute to the behavioral effects of indoleamine hallucinogens', Neuropharmacology
2024
In a controlled study (N=17), ergotamine, another migraine drug, given before psilocybin did not significantly alter the subjective experience; no triptan study exists. Halman et al., 'Drug-drug interactions involving classic psychedelics: a systematic review', J Psychopharmacol

Safety guidance

  • Low practical concern for most people.
  • More caution with heart disease, high blood pressure, or past stroke, where triptans are already restricted.
  • A hard trip can bring on a headache; using your usual triptan dose is not known to be dangerous, but avoid stacking extra doses.
Do not stop a prescribed medication on your own. Stopping or skipping a prescribed drug to take mushrooms can be more dangerous than either alone. Talk to whoever prescribes it.

When to seek help

Call emergency services for chest pain or tightness, sudden weakness or numbness, slurred speech, or a sudden severe headache unlike your usual migraine. Emergency red flags.

Common questions

Can I get serotonin syndrome from a triptan and mushrooms?

No case is reported, and even triptan plus SSRI serotonin syndrome turned out to be rare in a large study.

Can I take a triptan for a headache after a trip?

There is no evidence of harm, but if you have heart disease, follow your usual triptan restrictions.

Does a triptan weaken the trip?

Unknown. A related migraine drug, ergotamine, did not change psilocybin effects in a small study.
