Psilocybin and benzodiazepines

Benzodiazepines calm a difficult mushroom experience and are the rescue medication of choice in research guidelines, but they bring sedation, memory gaps and dependence risk.
Reduced anxiety and intensity, drowsiness, and patchy memory of the period afterwards. This is why it is the standard rescue medication in clinical research.

Why it happens
Benzodiazepines enhance GABA-A signalling and cause sedation and reduced anxiety without acting on 5-HT2A, so they dampen distress rather than reverse psilocin's receptor action. No controlled study has measured how much they reduce psilocybin's perceptual effects.
What is known
Clinical research safety guidelines name a benzodiazepine as the pharmacological intervention of choice when reassurance alone does not settle acute distress, while stressing that medication should rarely be needed.
- 2008
- Safety guidelines for human hallucinogen research state that when acute distress is not managed by reassurance, a benzodiazepine is the pharmacological intervention of choice, suggesting 10 mg oral diazepam, while noting medication should rarely if ever be needed. Johnson, Richards & Griffiths, Journal of Psychopharmacology 2008
- 2022
- A systematic review found no studies of benzodiazepines used together with psilocybin or MDMA. Sarparast et al., Psychopharmacology 2022 (systematic review)
- 2026
- The Valium label reports anterograde amnesia, with risk increasing at higher doses, and warns that abrupt discontinuation after continued use can cause life-threatening withdrawal reactions. VALIUM (diazepam) US prescribing information, DailyMed
Safety guidance
- Reassurance and a calm setting come first; a benzodiazepine is a backup when that is not enough.
- Expect drowsiness and patchy memory of the hours afterwards.
- Do not combine with alcohol or opioids, which add to sedation and breathing risk.
- If you take a benzodiazepine regularly, do not stop or cut it suddenly; talk to your prescriber.
When to seek help
Call emergency services if someone is very hard to wake, breathing slowly or shallowly, or has also taken alcohol or opioids. Emergency red flags.
Common questions
Will a benzodiazepine end a trip?
It usually reduces anxiety and intensity and makes people drowsy, but it does not switch psilocin off.
How much is used in research?
Research guidelines suggest 10 mg oral diazepam when reassurance alone fails; personal dosing should follow your own prescription.
Is it safe to combine routinely?
No. Benzodiazepines are dependence-forming, so they are best treated as an occasional rescue, not a routine pairing.
